IRA Change of Account Information Form
Changes of address, name or beneficiaries as well as adjustments to your automatic investment plan can be made with this form.

IRA Distribution Form
In order to redeem shares of your IRA, this form must be submitted.

IRA Conversion Form
To convert a traditional IRA to a Roth IRA you will need to complete this form.

IRA Transfer Form
The IRA Transfer Form will allow you to liquidate and transfer your IRA assets to Sit Mutual Funds from a different financial institution.
